This position is primary located at the Aiken-Barnwell Mental Health Center, 1135 Gregg Highway, Aiken, SC 29803.
This position services patients in their home and natural living environment in Aiken and Barnwell Counties.
Job PurposeProvides direct nursing care to patients assigned to the Intensive Community Treatment Program/ACTS in the community and will work in outpatient clinic as determined by needs.
Provides medical and psychiatric education and consultation as indicated to the patients of ABMHC and their families.
Majority of services will be provided in the patient's home and natural living environment in Barnwell and Aiken Counties.
Job Functions Maintains a bill-time rate of 623 hours annually (first year).
Assesses health, social, psychiatric, and medical needs of ABMHC patients.
Administers scheduled intramuscular medications to patients as ordered by ABMHC prescribers and monitors patients regularly for beneficial effects, side effects, and other medications related issues.
Provides medical, psychiatric, and nursing education and consultation to patients, caregivers, and staff.
Schedule clients for ICT by geographic area in order to maximize productivity hours.
Ensures patients are seen in accordance with their needs.
Completes clinical documentation using collaborative documentation.
Participates in clinical supervision and treatment team meetings.
Ensures quality customer service is provided to patients and stakeholders, complies with Corporate Compliance Policies and Risk Management Plan.
Pursues professional development through collegial consultation, continuing education, and licensure.
Ensures caseload size meets SCDMH guidelines.
Completes documentation according to SCDMH/QA standards.
Performs other duties as needed.
Minimum and Additional Requirements State Requirements:
Graduation from an accredited school of nursing.
Agency Requirements:
Licensed as a registered nurse by the SC Board of Nursing.
A bachelor's degree in nursing and two (2) years' experience; or a three (3) year diploma and two (2) years' experience; or an associate degree in nursing and two (2) years' experience as a registered nurse, one (1) of which must have been in a community health program.
Preferred Qualifications Agency (Facility) Preferences:
Licensed as registered nurse by SC Board of Nursing.
Employee will receive general supervision by the Chief of ICT/ACT program and consultation with Nurse Manager and Chief of Psychiatric Services.
Ability to operate a state vehicle.
Ability to lift 5 pounds.
Position is considered essential and may be called back during crises/disasters/emergencies.
Bilingual abilities in English and Spanish (or another language) are a plus.
